The team
Simcart Limited specializes in delivering specialized services and cutting-edge solutions in the field of medicine. As part of our portfolio, we own and operate several software applications, notably RenalReality and DiabetesReality. These Reality Apps are meticulously designed and constructed using a robust multi-tier architecture comprising of multi-tier MVC/WCF/SQL Server stack, with data seamlessly imported from patient management systems and pathology results databases via regular SSIS ETL extracts. Furthermore, our applications are intricately configured to access and retrieve data from the DXC WebPAS and Sysmex Eclair diagnostic results repository.

HealthCART: the mobile application
This mobile phone application is poised to change healthcare by providing doctors, nurses, and patients seamless access to Diabetes and Renal Reality web-based data and personalised care plans through smartphones and other mobile devices.
For healthcare practitioners, integrating mobile information technology represents a significant advancement, as it extends the availability of vital resources right to the point of care and even redefines where that point of care can be located. This application prioritises users' needs by granting them widespread access to pertinent information and enabling remote data capture. Consequently, users are no longer tethered to a physical network or confined to a particular location.
The Diabetes and Renal Reality smartphone application named HealthCart empowers patients by delivering tailored information and guidance directly to their fingertips. Utilising user-friendly interfaces, patients can effortlessly share their healthcare data, receiving relevant advice tailored to their specific conditions. Given that diabetes and chronic kidney disease are conditions characterised by numerical metrics, this application excels in generating precise, customised decisions for each patient. It empowers individuals to take control of their diabetes and CKD management with unprecedented convenience and ease, all from the palm of their hand and not restricted to a specific geographic location. Diabetes and Renal Reality smartphone application (HealthCart) can deliver patient-focused information and advice, as demonstrated here. With user-friendly displays, patients can share their health care information with the appropriate advice delivered parallel to their disease. With diabetes and chronic kidney disease being number-driven chronic conditions, this application will be able to make clear-cut decisions individualised for each patient, allowing them to take ownership of their diabetes and CKD management from the palm of their hand.

Diabetes Reality is a software application created to assist in the management of diabetic patients. It covers every stage of diabetes, from the initial engagement with medical services to conservative care. The application was developed by Dr. Allister Williams, a renowned consultant renal physician with impressive qualifications, including MBChB MPhil from Stellenbosch, MRCP from London, and FRACP from Australasia.
The user interface of Diabetes Reality has been meticulously designed to align with the mindset of diabetic patients. The application was launched on November 27, 2022, at Te Whatu Ora Taranaki in New Zealand, and has since received valuable feedback from users.
In an effort to improve service delivery, we are planning to implement PowerBI dashboards to render data "live and interactive" at the service level. This strategic move will enable healthcare providers to manage diabetes within our community more effectively, gaining valuable insights into the progress and outcomes achieved within our region.

Our vision with Reality Apps "helping hand in health"
"Patients taking ownership of their Chronic Kidney Disease (CKD) can allow for a cost-effective method of managing both progression of CKD and overall maintenance of patient well-being." HINZ 2014
However, as with all other medical practitioners, we need to adapt and use the advances made in Information Technology to deliver a platform to the patient through which the patient can share personalized healthcare information, enabling them to take ownership of their CKD disease through individualized care plans - called Automated Personalized Plans-of-care (APPs)
This platform should consist of automated information, advice, and guidance about the progress of their kidney disease and the associated organ-specific complications. Information technology (IT) advances enable this redesign of healthcare processes based on using and integrating electronic communication at all levels. New communication technologies can now support a transition from institution-centric to patient-centric applications and allow patients and clinical staff to take renal and diabetes care into their own hands.
