Built with the Taranaki Diabetes Service Level Alliance to improve outcomes against Ministry of Health standards — combining primary and secondary data, podiatry pathways, psychosocial screening, and standardised risk assessment.

  • MoH-aligned diabetes standards. A single register and shared care planning model spanning iwi, primary care, and specialist services.
  • TRAFFIC clinic optimisation. Colour-coded care needs routed to the right clinician, place, and time — tracked against real service capacity.
  • HealthCart patient app. Automated CarePlans and alerts reach patients within an hour of bloods or consults.
  • PowerBI MoH dashboards. Live KPI views for decision-makers, aligned to New Zealand Ministry of Health diabetes care requirements.
  • Clusters and allocations. GP-practice clusters, ethnicity filters, and clinician caseloads keep community teams organised.
  • Diabetes in pregnancy screening. Opportunistic alerts from HbA1c and HCG combinations to surface gestational diabetes risk earlier.
