step 16: Reports - monthly RRT summaries
This section prepares reports for monthly MDTs per modality and per renal parameter. The last four months of results and the reason for alerts will appear here as demonstrated. The Alert status (Red firsts, then Orange, then Green) appears on the left-hand column. You will also notice other RED TRIGGERS associated with the patient. When you hover over the patient's name, the reason for this particular trigger will appear. When you click on the NHI, you can go to the patient summary page with a summary of all alerts will show. Each RRT modality is reported separately. Within the RRT modality, anemia, and CKD MBD split. eGFR trends reported as part of Bone/GFR reports. The current treatment (EPO/Iron) is displayed per patient. These lists can be reorganized according to Color Alerts and Filters.
Last 4 results and Mean values displayed
- For Anemia, the variables Hb, Ferritin, and TSAT% displayed
- For CKD MBD, PTH, Calcium, and Phosphate levels were displayed. eGFR also appears here.
- The mean value for each is displayed.
Each modality reported on CKD MBD and Anaemia
- Two lists appear for each RRT modality, one for Anaemia, one for CKD MBD/eGFR
- eGFR trends appear with CKD MBD
- Each Red Trigger associated with the patient is shown in Red underneath the patient's NHI
Alerts displayed on the left side of the page
- This allows the clinician to attend to those patients in RED first.
- Each Red trigger (irrespective of the Alert color) is also displayed to make you aware of it
- Click on the NHI takes you to the patient page
Step 17: reports - how to search for specific events
From time to time, you will need a specific set of data for research, auditing, or clinical reporting reasons. You can use the SEARCH button at the top RIGHT SIDE of your Menu items.
Under the SEARCH button, you can build any combination of results if you know how it's stored/granulated. This is quite difficult since you need to know the actual granulation code used - which will allow you to look for that particular dataset. For example, if you need to know how many Diabetics started dialysis in the past three years, you can find the number and actual NHIs/patients - plus their current modality.
After you find your data set, you can export data to MS Excel, where you can process the data further using all the features in MS Excel.
Like all other patient lists, the data can be filtered/sorted using the columns
Event granulation
- RRT = renal replacement therapy
- HD = Haemodialysis RRT modality
- PD = Peritoneal dialysis RRT modality
- TPL = transplantation RR modality
- CKD = Chronic kidney disease
Export to MS Excel
You can export your data to Excel
- The Red number next to each RRT modality indicates the number of Red Alerts
- You can click on any of these numbers, and you will be negotiated to the actual Red Alerts
- These Red Alerts recorded on a timeline
Current RRT Modality
Each NHI event matched to current RRT
- The right-hand column shows the current RRT modality for the patient that met the Filter criteria of your search.
- Events can be sorted and filtered using the content of the RRT modality.
Step 18: reports - finding critical events
This is a convenient feature when you have been away from your unit for weeks or months. You can find CRITICAL EVENTS under REPORTS as the most Right-sided menu item under REPORTS. This list can be sorted/filtered as per the associated clinician.
REPORTS --> Critical Events
After you click on CRITICAL EVENTS, you can select the date range at the LEFT-hand side of the screen. The software developer can change the events you are interested in:
Critical events include:
1. Patient deaths
2. Dialysis Access changes
3. Modality changes
4. Access complications
Find critical events
Under REPORTS to the Far Right screen
- Go to the section REPORTS at the top of your screen and look to the Far-right
- The critical events MENU button appears there
- Click on it, and your screen will show a default set of critical events from the past week.
Filter/Sort your list
Using Columns to manipulate your list
- Each column can be used to sort by heading, alphabetically, or using keywords
- The date range can be changed on the top right-hand side of the screen
- Critical events can be sorted per Clinician
Export your data to MS Excel
Like SEARCHED events use MS Excel
- After filtering/sorting your critical event list, you can use the top LEFT MS Excel button
- This will allow you to export data to a spreadsheet for further analysis
- Critical events can be changed at the Admin level
Step 19 - reports - your personal caseload
Each patient is associated with a Clinician. The CaseLoad shows patient per Clinician. By clicking on CaseLoad at the top of the LANDING page, the patient list will appear for the clinician. The Clinician can be changed at the top left corner.
Whilst on the CaseLoad page, you can re-organize the patient list using several filters and sorting mechanisms. Lists can be sorted using location, modality, or ethnicity. You can also Sort the patient list as per any of the top menu items, like name, eGFR, Age, and Gender.
Return to the Home Page by clicking on the RenalReality icon at the Top right of your screen. This applies to all screens you might be on.
Enter CKD Cause and Medical Background
- Go to the Headline tabs and click on CKD Cause
- View CKD Cause and Medical Background here
- Click on NEW EVENT and drop-down list to select Cause of CKD
- Using the first letter of the CKD Cause, you can find the item quickly
- Copy and Paste or Enter the Medical Background in the Free Text Column
Start and Changes in RRT Modality
- Go Headline tabs and click on Current RRT Modality.
- Alternatively, you can go to NEW EVENT --> Change in RRT modality
- Click on New EVENT and drop-down list all your options
- Select your tertiary and quaternary options with drop-down choices
- In the Free Text column, you can type any additional comments
Review and Update patient medication list
- Go to the ADVANCE MANAGEMENT tab at the bottom of the Summary page
- Here you can view and scroll through current and recent changes
- Click on New Event to change the medication list (Summary List)
- Copy and Paste or type the list of medications and dosages
- Click Save or Cancel to change – this can be viewed/edited under Clinical data
Update Dialysis scripts and sessions
- Go to the ADVANCE MANAGEMENT tab at the bottom of the Summary page
- It depends on the current RRT - the most recent script can be viewed here
- Click on New Event to change the RRT script
- Each column can be completed using a preselected drop-down or through free text.
- Click Save or Cancel to change – this can be viewed/edited under Clinical data
View and update Transplant status
- Go to HEADLINE tabs and click on Transplant status
- Here you can view and scroll through current and recent changes
- Click on New Event to change the Transplant status
- Using the drop-down options for tertiary and quaternary selections
- Click Save or Cancel to confirm your selection
Maintain patient and staff associations
- Go to the ADVANCE MANAGEMENT tab at the bottom of the Summary page
- Here you can view and scroll through current and recent changes
- Click on New Event to change the medication list (Summary List)
- Copy and Paste or type the list of medications and dosages
- Click Save or Cancel to change – this can be viewed/edited under Clinical data
