step 6: Patients summary page lay-out

RenalReality allows you to see the most relevant aspects of patient care at a glance. This is a very complicated screen and its important you learn to know how to view it, and how to use it. Here you will be given an extensive breakdown of the Patient Summary Page
  • Patient details at the top -demographics and RRT category
  • Automated color-coded CarePlans for each Result Grid
  • Result Grids - showing the last 8 monthly renal parameters
  • Metabolics for weights, diabetes, hypertension, and lipids
  • Advance Treatment tabs for Anaemia and CKD MBD strategies
  • THREE DOTS indicating Clinic visits and e-NOTES
  • Headline TABS of the latest STATUS on ACCESS and Transplantation
  • Links to NOTICEBOARDS and REMINDERS
  • Links to last USKUB and whether Parathyroidectomies performed
  • You can click on any column to access all results for that parameter
  • By hover over the renal parameters, the normal range for that patient within its RRT modality will appear on the screen
After the selection of your patient, you will enter the Patient Summary Page, with all the data of the patient as a dashboard ** in RR3 you can flick between Daily (Transplant)( vs Monthly Dashboards
Patient details
Patient name, NHI and RRT status
  • After patient selection, you will see a Summary Page of the patient appears
  • All the relevant patient data on one screen gives you a bird's eye view of the patient
  • Every possible data entry possible from here.
Color coded CarePlans
Alerts with Red, Orange and Green lines
  • Each Result grid show a color-coded bar on top which represent an Alert Status
  • The Alert status indicates the severity of the care plan for that particular Renal CarePlan
  • You can access these alert with a click
Result Grids
Last 8 sets of monthly blood results
  • Each of the 4 Result Grids contains the last 8 months of blood results for your patient
  • Only the last result for each month appear
  • ** in RR3 you can switch this feature to show daily vs monthly results in these grids

step 7: Patients using the Headline tabs

Just under the patient details, you will find the Headline Tabs. The most relevant data and dates appear here. To see a full history of each tab, you can click on it. When the window pop-up with the history of clinical events, you can enter a new clinical event for this section.
  • Current RRT Modality, ie Haemodialysis and when started
  • RRT plans and dialysis sessions with a reference date
  • RRT Access status - most recent status and when updated
  • Cause of Chronic Kidney Disease - plus a medical background
  • Transplant status and when last updated
  • Primary contacts - both Clinicians and renal nurse contact
  • Notice Board - and reminders of upcoming events posted
Headline Tabs show you the most recent updates/status of important renal aspects. **in RR3 NoticeBoard will display any unattended reminders posted by users
Patient associations
Each patient can be associated with different renal staff
  • Click on Patient contacts in the Headline tab
  • A form will open up where you can add and delete current patient associations to renal staff
  • This staff assocication generate CaseLoads
Renal Replacement Therapy
Plans, Current modality and Access
  • Current RRT modality appears in the first Headline tabs with date started
  • Future RRT Plans, or current RRT sessions and Resuscitation status updated in 2nd Tab
  • Current RRT Access shows in next Tab - with all previous records popping up with a click
CKD Cause and background
Showing the recent for CKD
  • This will show you the patient cause of CKD which is relevant in clinical decision making
  • You can also view and edit the relevant medical background in free texts columns when you open this Tab on a click
** in RR3 you can customize the Summary Page based on whether you want daily vs monthly results grids ** in RR4 you can choose which Advance Management Tabs you prefer the Summary Page to Default to ** in RR3 the NoticeBoard will show unchecked Reminders for this specific patient with a numeric Red

step 8: Patients notices and reminders

To the right of the Headline Tabs, you will see the Menu Item NOTICEBOARD. This contains a list of Notices, Reminders, Most recent events, and CarePlan updates.
Upcoming events (Reminders) are clinical events posted with a future date CarePlan update is purely a date when this had been updated Previous events put in chronological order the most recent event including: 1. Surgical renal procedures 2. Modality changes 3. Clinical events like meetings and OPD clinic visits 4. Modality changes 5. Transplant updates
All these events allow you a link to the actual event
The NoticeBoard gives you quick access to Reminders, Alerts and recent event entries **in RR3 you will be able to QuickLink to these entries made by users and displayed here **Careplans giving you a link to patient current CarePlan page
About the Notice Board
On the most right side of the Headlines
  • Within the NoticeBoard box, the most recent event/reminder will appear with a date
  • This will remind you of any upcoming events posted by users for this patient
  • Anyone can post a reminder, whilst care plans and events are automatically posted
  • ** in RR3 a QuickLink will take you to events
How to add Reminder
Always use future date for reminders
  • Any secondary event can be added to the notice board through the MenuTree
  • Make sure you use a future event date which will automatically be added to reminders
  • In RR3, these reminders will appear in Red when it becomes overdue
Links to the actual event*
Details of reminders can be accessed
  • Go to NoticeBoard on the most Right-hand side of the Menu Bar
  • Click on any of the listed events
  • *This feature only available in RR version3
  • You can then access the actual posting in terms of notes and the user that posted it

Step 9: patients advance management

At the bottom of the patient summary page, you can access advanced management tabs on your patient. This includes the following: 1. Anaemia management (EPO and Iron) 2. CKD MBD management (Binders and Vitamin D) 3. Dialysis scripts for Haemodialysis and Peritoneal dialysis 4. Medication lists 5. Transplant workups 6. CarePlans
You can click on any of these Tabs to view and edit the latest Advance management strategies for this patient, whilst having a view of the Summary page on the background. These forms can be viewed and moved over the Summary Page
Advance Management strategies can be accessed and viewed here. **in RR3 Protocol driven Treatments like Lupus protocols can be linked/planned for patients
Anaemia and CKD MBD
Click on Clinician to sort by name
  • For anemia EPO and Iron, scripts can be entered here by nurses
  • For CKD MBD Phosphate binders and Vitamin D supplement options can be entered
  • View and Edit option available for all Tabs
Dialysis scripts
PD and HD script forms appear here
  • Dependant whether the patient is on HD or PD the appropriate script option will appear
  • The last updated script will appear as the default whilst you can scroll through previous scripts entered and updated by users
Summary of CarePlans
Summary of all the Careplans appears
  • Click on CarePlans at the most right
  • A list of the 6 renal categories each with each own Alert color and care plan.
  • At the top, the date for the next set of renal bloods as per recommended by guidelines

step 10: patients important alerts

Medical alerts and Concerns appears next to the patient name as a Red or Red icon. Medical Alerts are mainly microbiology results listed below. If green these icons will appear green. All social concerns will appear in Red.
To view more details of these alerts you can click on them and a pop-up will appear that shows more details about these alerts and concerns.
Relevant scans (USKUB) that you need to be aware of appears on the top of the Biochemistry Result Grid. Similarly, on top of the Bone disease result grid, previous parathyroidectomies will appear here, so that the clinician can interpret the CKD MBD result in the correct context. **in RR3 a QuickLink to Renal Biopsy reports will be available
Important Alerts about Virology and Social concerns showed here. **in RR3 Alerts about Specialist Authorities (like EPO) will appear here
Virology
Positive results in virology turns RED
  • When a green icon appears, it means all results checked in this section NEGATIVE
  • For any POSITIVE result, a RED icon will warn the user to click on it for more details
  • Details of the result not visible for privacy rules
  • ** in RR4 each unit can set the Alerts required
  • ** in RR3 Alert to Renal Biopsies will be added
Social alerts
Social concerns raised by staff
  • Users that entered reasons for concerns especially about social issues will be highlighted here to make users aware of it
  • If it's RED you can click on it to view details
  • If no concerns raised, no icon will appear
  • As we mature the RR product this Alert can also include current Acute Admissions
USKUB and PTH Surgery
Each GRID with a relevant notice
  • For Biochemistry the last USKUB if done, uploaded from Eclair will show here
  • Same with CKD MBD where workup and completed Parathyroidectomies will show
  • ** in RR3, these link to the actual event
  • ** in RR3 anaemia Grid will show EPO alerts
