step 11: Events using quicklinks
- RenalReality allows the entering of the most common clinical events under the heading: New Event - QuickLinks:
- Cause of CKD - Medical background also added/updated here
- RRT plans - the choice of Modality preferred
- OPD clinic notes - every visit associated with e-notes
- Medication updates - every visit, clinicians review medications
- Access-related entries - for all your vascular access entries
- Changes in RRT modalities - including transplantation
- Transferring care between units - moving to and from
- Renal procedures - renal biopsies, parathyroidectomies
- Patient observations - BP, weight, BMI, etc
- Patient deaths - although automatically updated, manually done here
- Family meetings - outcomes/notes from family meetings
- Home visits - FORMS used to complete this section
New Clinical Events
- Click NEW EVENT in your Menu Bar
- Your QuickLinks options will drop down for you to choose from - this can be changed.
- After making your choice, you will be taken to the MenuTree, where you can enter and granulate your entry into finer detail.
Event Granulation
- Each clinical event in RenalReality is fully granulated in a predetermined MENUTREE.
- You must familiarize yourself with all the thousands of options and events over time.
- Event granulation is done so that each event is fully "bar-coded" for future searches.
Changing the QuickLinks
Can only be done at Admin level
- Your top menu bar will include ADMIN if you got admin level rights
- Click on it, and you will see the option to create and update QuickLinks (New Events)
- A list of current links will appear, and you can then add each level of your new QuickLink
Step 12: events using the menu tree
At the top of the Summary page, clinical data is organized under the following headings
1. Summary - this is the Dashboard or Summary Page of the patient
2. Encounters - patient events when in contact with renal service
3. Access - dialysis access
4. Results - blood results
5. Clinical data - scripts and medications
6. Patient notes - electronic notes entered by users
7. All events - summary of all data entered, accept blood results
8. Forms - here, you can enter predefined forms
9. Documents
10. New Events - Quick links
When in the patient Summary Page you can view historical events using these tabs or you can look at all event using the ALL EVENTS menu option
The MenuTree
- Primary - your main menu items as above
- Secondary - submenu items per primary item
- Tertiary - options defined by each tertiary level
- Quaternary - the finest level of granulation within each of the tertiary levels
- For example, ENCOUNTERS (Primary) followed by Transplant Status (Secondary), then Recipient (Tertiary), then accepted (4th level)

- As you click your menu choices, the different submenu options become available to you
- Each Menu level gives you the next level of alternatives until you finalize/granulate your choice
- Now you can change the date
- With each clinical event, you got the option of a free text column to add comments.
- This completes the entering of clinical events.
Deleted/Edit existing Data
Full audit trail kept as you edit events
- Use the EDIT button in the Actions column to delete an existing clinical event.
- Change the Valid (non-valid) action button to delete an event or choose to show VALID vs. NON-VALID events in the Event list.
- Edit notes by clicking on the column, allowing you to change the notes or comments completed by other users.
Step 13: events using forms to enter new events
Using the FORMS section, you can enter data against a preset list of parameters and options as designed by your local unit.
Each of these FORMS will have a date for you to change, pending when this event happened. The default date will always be today's date.
Some of the Parameters will have predetermined options as a drop-down box. To the right of each set of options, you can sometimes add free-texted notes to clarify your options.
These parameters and options can be changed at the ADMIN level.
You must click the ENTER button to confirm the data as above. These forms can be edited afterward or accessed through timelines. Scrolling options with numbers appear at the bottom of each form over time.
Using FORMS in general
Generic approach same for all FORMS
- Make sure you enter the correct date.
- For each parameter, click on your drop-down options that will appear in a box
- Click on each option and move on
- Enter notes/comments if required
- ** In RR3, these forms become Basic Functional units with Action Buttons

- After saving a FORM, finding it uses a numeric pattern at the bottom of forms,
- Previous data items are entered this way using this form accessible through this method.
- When editing these forms, a new one is formed so that you keep a record of all forms.
Changing FORM lay-outs
This can only be done at ADMIN level
- New forms can be created at the Software developer level by contacting Simcart Ltd
- The layout of existing FORMS can be changed, including the options of each FORM
- You need ADMIN-level access to do this
- FORMS will eventually have generic options
Step 14: events adding attachments**
Sometimes you got data stored in another format you would like to add to an existing clinical event in RenalReality. For example, a photo of a rash or a diagram of vascular mapping was done in preparation for AVFistula creation.
File formats supported as part of RenalReality software application
1. Word documents
2. Powerpoint presentations
3. Documents stored as images (png, jpeg, bmp)
4. PDF documents like clinic letters or referrals
After entering events and adding attachments, this can be accessed via the menu items at the top of your page. It's best to name our files so you can see the named descriptive detail of the file attached to the clinical event. You can delete files or attachments as you delete events.
* This feature is now available in version 2.7 onwards
How to attach a file
The Clinical Events must be entered first
- Go to the existing clinical event using the Top SubMenu that categorizes each event
- Click on the ACTION button that will give you the option of ATTACH A FILE
- You can now select and add a file from your PC
How to acces attachments
Just like you find stored Clinical events
- In the same way, you find any clinical event; you negotiate your way to the events and files.
- If a file is attached, you will see under the column files and the descriptive name you use
- Click to download or delete the file if required.

Using menus you can look at files
- In your top Menu, you can see the option FILES
- Click here to access all files ever attached to your clinical database
- Click on the file to open or edit it. You can also delete files here with a full audit trail.
How to enter CKD cause and medical background
All patient must have a CKD cause and Medical Background
- Click on NEW EVENT (QuickLink) ---> Cause of CKD
- Click on Category to expose your list of options in the drop-down box
- Scroll down or use the FIRST LETTER to find your option
- Click on your selected option and move to the Free text box
- Copy and Paste or Type Medical Background in the Free text box
- Make sure the date in your Calender box is correct - Default = Today
- Click on SAVE to store your selection or CANCEL to exit
How to enter RRT PLANS which RRT modality?
Leading up to ESRD each patient must have a chosen Modality including conservative management (not for RRT)
- Click on NEW EVENT ---> Renal Replacement Therapy Plans
- Click on Category to expose your list of options in the drop-down box
- Scroll down or use the FIRST LETTER to find your option
- Click on your selected option and move to the Free text box
- Type in comments that support/explain the choice made
- Make sure the date in your Calender box is correct - Default = Today
- Click on SAVE to store your selection or CANCEL to exit
How to enter dialysis access changes
Before starting renal replacement therapy, each patient must have Dialysis access which can be entered here. Access complications can also be entered here.
- Click on NEW EVENT ---> Access related entries
- Click on Category to expose your list of options in the drop-down box
- You can select between Create, Remove, or Problems for any complications associated with current access
- After selecting the secondary option, you can now select the tertiary access or complication associated with the second choice
- Type in comments that support/explain the choice made
- Make sure the date in your Calender box is correct - Default = Today
- Click on SAVE to store your selection or CANCEL to exit
How to update patient RRT MODALITY
When the patient is ready to start a new modality like dialysis or transplantation, you need to update the RR records using these steps
- Click on NEW EVENT ---> Change RRT mode
- Click on Category to expose your list of options in the drop-down box
- You can select between options exposed in the drop-down list
- After choosing your option, you can now type in comments that support/explain the choices made
- Make sure the date in your Calender box is correct - Default = Today
- Click on SAVE to store your selection or CANCEL to exit
- Conservative therapy can be entered here or via RRT Plans
How to update patient transplant status
Updates during workup towards a kidney transplant, including having a kidney transplant can be entered here - using the steps below.
- Click on NEW EVENT ---> Transplant Status
- Click on Category to expose your list of options in the drop-down box
- For general notes: Use the option --> Transplant notes
- To update RECIPIENT status, select the option --> Recipient
- To update your DONOR status, select the option --> DONOR
- From here, you can pick an option as per the national transplant codes for updating DONORS and RECIPIENTS
- Click on SAVE to store your selection or CANCEL to exit
